Thalamotomy & pallidotomy

Lesioning creates a small permanent area of change in a selected brain target. Thalamotomy may reduce tremor. Pallidotomy may help selected movement symptoms, including involuntary movements associated with Parkinson’s disease.

How is the treatment planned?

The team uses detailed imaging and specialist assessment to choose an appropriate target. Radiofrequency lesioning uses a fine probe; focused ultrasound is another way to create a lesion for selected indications. The advantages and limitations are compared with DBS.

Benefits, risks & follow-up

There is no implanted device to programme, but a lesion cannot be reversed. Treatment is often directed at one side, depending on symptoms and risk. Speech, balance, sensation or strength may be affected. Bleeding, infection and other serious surgical complications are possible. Outcomes and recovery are individual.
